Medical Document Requests

Access Requests for Personal Health Records

If you require a copy of your personal health records from Health and Wellness Services, Western University, for any purpose (e.g., transferring records to a new family doctor) please complete the following:

Please Note: Processing Fees May Apply

  • Requests for personal health records sent directly to the patient will incur a fee of $30 for pages 1-20 and an additional $0.25/page thereafter. The final cost of processing is dependent on the total number of pages released and will vary with each patient. The patient will receive an estimate of the fee by email correspondence.
  • The applicable processing fee must be paid in full prior to the release of the requested records.
  • Requests for personal health records sent directly to another health care provider will not incur a processing fee.
  • All other third-party requests for personal health records may be subject to the processing fee of $30 for pages 1-20 and an additional $0.25/page thereafter.

Medical Note Requests

Requests for notes are an uninsured service and appointments booked for this purpose are not medically necessary, therefore a fee for the visit and the note is necessary. Please check your course outline or confirm with your academic advisor that you need a medcial certificate prior to requesting one.

Where a note request occurs after the illness, our providers do not have a method to verify the authenticity of the absence/illness and may:

  • Decline completion of the notes/documents
  • Provide a document indicating the patient was not assessed but reports to have been absent with illness.

Medical Certificates can be obtained from Student Health & Wellness Services for a fee. For a full list of fees, please visit our Fees page.

  1. At the time of illness, make an appointment with a physician at Student Health & Wellness Services.
  2. During this appointment, request a Student Medical Certificate, from the Physician.
  3. Go to Student Health & Wellness Services to make your payment and pick up the form. Payment arrangements must be determined prior to receiving completed documents.
