Key/Card Access Requests

For all key/card activation requests please follow the steps below.

STEP 1 - Get Authorization

  • Key/Card Access request form (Excel file).
  • Have your SUPERVISOR email this completed Excel file to Vicky Lightfoot.
  • Undergraduates (Work Study or WVP volunteer) must have confirmed appointment approval before keys are issued or cards activated – Supervisors must attach the email showing confirmation of appointment approval and make sure the “end date” reflects the end of contract.

STEP 2 - Request a key

To request a key (not card access) from Facilities Management use the button on this page: http://www.uwo.ca/fm/client_services/keys.html (Note: We cannot currently request card access on-line. As long as card access requests are included on the Excel file (step 1) they will be passed on to the police to process)

  • and how to contact the Keys office

Please note: Delays may occur during peak times (September and January)

Teaching Equipment Inventory

An inventory of teaching laboratory equipment has been prepared for the information of researchers who may wish to use items when not in use. The equipment may only be used by members of the Biology Department. The inventory includes contact information for those responsible for the equipment. Researchers who wish to borrow equipment should ask the supervisor or the principal investigator in their laboratory to make the loan request. A loan agreement form is available which must be completed and given to the person from whom the equipment is borrowed. The supervisor or P.I. will assume responsibility for the condition and return of the equipment.

Obviously, some equipment is not portable so arrangements can be made to use it in situ. Other pieces of equipment, like balances, that may become unreliable when moved may also be used in situ.
