Request for Degree/Module(s) Change or Withdrawal

This form is used to request changes to your degree/module(s) in the following situations:

  • you forgot to complete your Intent to Register
  • you need to change the degree/module(s) you selected (to provide you access to appropriate courses) during Intent to Register
  • there is an error based on the adjudication process in May
  • your eligibility has changed due to summer results and you need to be in a specific module to have access to required courses
  • you were not here in during the 2011/2012 academic year and are returning to Western for next academic year

Please complete ALL sections of the form in any of these situations.

WITHDRAWAL from Western:

If you wish to WITHDRAW after requesting a line of registration for 2011 fall/winter, please complete section 1 and 3 of the form.

Please fax form to the Dean's Office (FAX: 519 661-3703) or drop it off at the Counselliing Desk, Western Science Centre, Room 191 during regular office hours.
