The Basic Steps Involved in Requesting Academic Accommodation:

  1. Review WebCT and/or your course outline to look for any comments from your instructor regarding accommodation.  Students seeking accommodation for work worth less than 10% of the overall grade for the course: instructors may oversee accommodation requests for participation, small assignments, etc.  Check your course outline or Web CT as it should indicate whether accommodation should be arranged through your instructor or through the academic counselling office. 
  2. Send an e-mail to your instructor or course / lab coordinator to inform them that you missed a course component (unless WebCT or your course outline suggest otherwise).
  3. Complete an Accommodation Request Form if you will miss a course component due to:

Request accommodation through your home faculty academic counselling office (located in WSC 191, for Science and BMSc students).*  Submit Accommodation Request Form (if applicable) and documentation (not applicable to religious accommodation requests).  Once an accommodation request has been approved, the member of the academic counselling team who approves your accommodation request will inform your instructors of our support for accommodation.  If a student is accommodated for a December or April final Exam, they will be issued a Recommendation for Special Exam form instead, which the student should bring to their instructor.
*Please do not visit our office until you are well enough to come to campus; do not visit our office if you are contagious.  If you are not able to submit your accommodation request in person within a few days of the missed course component, please email our academic counselling team about your situation.

About Medical Documentation

Documentation can be issued from a physician or type of health care professional; this will depend on the circumstances of and type of illness.

Obtaining Documentation from Student Health Services:

  • Students planning to request accommodation should request medical documentation when they see the doctor
  • If the doctor supports academic accommodation, the student will be provided a Student Medical Certificate

Obtaining Documentation from Walk-in Clinics and Family Physicians:

  • A Western Student Medical Certificate (SMC) is required where a student is seeking medical academic accommodation. This documentation should be obtained at the time of the initial consultation with the physician or walk-in clinic. Note that most walk-in clinics in London will have SMCs available, but for family physician visits, you should bring an SMC with you.

Obtaining Documentation from Urgent Care or Emergency Department:

  • Students are encouraged to request that the doctor complete a Student Medical Certificate. Students should bring this form with them, if possible.  Where it is not possible for a student to have an SMC completed by the attending physician, the student must request documentation sufficient to demonstrate that his/her ability to meet his/her academic responsibilities was seriously affected.  Documentation must be legible and indicate the time frame involved, so it is evident that the illness and doctor's visit was before or at the time of the missed course component.
