Description:

The Healthy Communities Fund (HCF) grants project stream is a one-window approach to funding local organizations for the delivery of health promotion initiatives related to: physical activity, sport and recreation; healthy eating; tobacco use and exposure; injury prevention; substance & alcohol misuse; and mental health.

The Grants Project Stream is a cost-sharing program designed to help eligible organizations create opportunities and mobilize communities to become healthier. The program provides cost-sharing support for non-capital projects that will contribute to the health and wellness of communities. A ?community partner? must be involved in the planning, development, implementation and/or evaluation of a project. All applications must have at least one community partner that will assist with these phases of project delivery. Projects in communities with a population greater than 20,000 must make a matching cash contribution of at least 15% of the requested amount from the HCF Grants Project Stream and a matching in-kind contribution of at least 50% of the requested amount from the HCF Grants Project Stream. Projects in communities with a population of 20,000 or less must make a matching cash and/or in-kind contribution of at least 25% of the requested amount from the HCF Grants Project Stream.

Application Details:

Eligible organizations are required to work in partnership and take an integrated approach to improving health and wellness at the community level. By working in partnership, organizations have an opportunity to gain new expertise and to reach a broader audience while providing more comprehensive programs to Ontarians in their own communities.

All applicants should read these guidelines and must consult with a Ministry Regional Advisor at the earliest possible opportunity and before completing a grant application form. Additional details pertaining to contact information and the purpose of consultation can be found under the section entitled ?How to Apply, in the program guidelines. The HCF Grants Project Stream aims to support local/regional initiatives that impact the health of an underserviced community or population.

INDIRECT COSTS ARE NOT SUPPORTED BY THIS PROGRAM
