Description: |
The overall purpose of the PEP initiative is to promote and support the engagement of patients in the design, delivery and evaluation of health services that lead to quality patient-centered care. The PEP initiative supports one of CHSRF?s three strategic priorities ? engaging and supporting citizens. |

Application Details: |
Participating teams are required to secure matching co-sponsorship support at a 1:1 level (cash and/or in-kind). Teams recommended for funding are also required to participate in a learning initiative coordinated and supported by CHSRF. Over the course of the project period, all funded teams will be brought together three times to take part in workshop learning sessions, where they will learn from key experts and from one another. The application form must be requested online. An original and two full copies of the completed, fully signed application must be received at CHSRF by May 27, 2010 at 12 p.m. (noon) eastern standard time. In addition to the original, e-mail an electronic version of the completed application form in MS Word format (excluding attachments) to grantsandawards@chsrf.ca.INDIRECT COSTS ARE NOT SUPPORTED BY THIS PROGRAM. |

NOTE: Letters of Intent and Full Applications are to be processed through Research Development Services for institutional signatures, and are to be accompanied by a completed RDAF form (bearing applicant, Chair and Dean signatures). Requests for Institutional signatures should be received no less than 3 days prior to the posted Sponsor deadline. |
