Nominations

The Western Award of Excellence is the highest level of campus-wide recognition for the exceptional performance of Western staff (individuals and teams).  Nominations for the 2013 awards are being accepted until December 10, 2012.

Open and save the nomination form as a word document. After you have completed the nomination information and are ready to submit please forward the document by email to staff.recognition@uwo.ca

Electronic submissions in word document preferred. However electronic PDF and hard copies are accepted. If submitting in hard copy please forward nomination to Staff Recognition Office Room Room 5100, SSB.
