This form is intended to be used to request information on your eligibility for leave and Supplemental Employment Insurance Benefits (SEIB) from Western.  It will also allow Human Resources to help you plan and prepare for your leave.  Please note: all information provided will remain confidential.

There are a number of factors that impact your eligibility for leave and benefits including the length of time you have worked at Western, your employee group collective agreement, and your employment status with Western. Please provide answers to as many of the questions as you can in the form below.  A Western Human Resources Benefits Consultant will determine your eligibility and contact you with the information you will need to plan and apply for your leave.
