In order to ensure the safety of students and patients, the following medical and non-medical requirements must be met by students prior to participating in a fieldwork placement.
These tables are to be used as guidelines by the student and their health care provider to obtain the necessary medical & non-medical documentation required for Western University OT fieldwork placements.

If only one previous vaccination, then one additional dose required before blood work completed.
|Hepatitis B||Documentation of immunization series dates:
3 doses series: 0, 1 and 6 months
2 dose series (given between 11-15 years of age): 0 and 4-6 months
(*Immunization series not required if blood test demonstrates Immunity)
Blood test indicating immunity (antibody to Hepatitis B surface antigen)
|Polio||Documentation of primary immunization series dates: (4 doses; 2, 4, 18 months then one dose after the age of 4 years).||If no documentation then adult vaccination series required.|
|Tetanus / Diphtheria / Pertussis||

|2 step TB skin test**
|Documentation of 2 step TB skin test once per life time.||If positive TB skin test, documentation:

You must have documentation of at least 1 previously negative 2-step TB skin test. If one of the tests has not been completed within the past 12 months, based on the last day of your 9581 placement, you require an additional single TB test.
For positive TB test reactors, a chest x-ray, current within the last 12 months, based on the last day of 9581 placement is required and a copy of the x-ray report is required. Anyone with a confirmed positive TB test and who has not received counseling or advice concerning prophylactic treatment should be referred for an expert consultation by their treating practitioner. In addition, those who have received counseling or advice concerning prophylactic treatment should provide a copy of their consult note.
|N95 mask testing||Documentation indicating test valid; to be completed every two years; must be one of mask types listed under type.
Testing arranged by the School of OT through 3M in Sept/Oct. for 1st yr. students.
|Criminal Record Check (CRC) with Vulnerable Sector Screening (VSS)||Documentation indicating valid for 12 months
18 Years of Age and Over
Obtain from Ontario Education Services Corporation (OESC). Review the information carefully. If you fit the OESC criteria for having them process your record check, then you must obtain an OESC ID card. You MUST select the OESC Option that has the written report. Contact OESC to confirm processing time or for more information about the ID card.
If you are not able to obtain an OESC ID card because you do not fit the OESC criteria for having the record check completed through OESC, then you will obtain a Police Records Check & Vulnerable Position Screening from your Local Police Service. When completing the request for Vulnerable Position Screening, note on the application that you are working in an area where there are children and/or vulnerable persons.
|First Aid||Document indicating first aid valid until the end of the program.||Obtain First Aid and CPR certification which is valid for a 3 year period.|
|CPR||Document indicating CPR valid until the end of the program.|
