Placement Injury / Accident Reporting:

Western University nursing students who are on a placement as a formal part of their course or program are covered under the Workplace Safety and Insurance Act and are eligible to collect Workplace Safety and Insurance benefits in the event of a workplace accident.

The School of Nursing will complete a WSIB Work-Place Education Agreement (WEPA) form for each student prior to each practice placement to ensure coverage in the event of an injury/incident; a student’s signature is required on this form. WEPA forms will be distributed at School of Nursing student orientations and must be signed and returned to the Placement Coordinator, Requirements & Information, Health Sciences Addition Room H138 prior to the start of each placement.

In the event of an injury/incident while attending a practice placement, the student must:

  1. Notify his/her clinical instructor or preceptor and receive first aid treatment
  2. Complete required reporting documentation for their placement site.
  3. With assistance of their clinical instructor, preceptor, or the School of Nursing Academic Counsellor, complete the Western University Accident/Incident Report form and submit, within 24 hours of the incident, to:
