Placement Injury / Accident Reporting:

Western University nursing students who are on a placement as a formal part of their course or program are covered under the Workplace Safety and Insurance Act and are eligible to collect Workplace Safety and Insurance benefits in the event of a workplace accident.

The School of Nursing will complete a WSIB Work-Place Education Agreement (WEPA) form for each student prior to each practice placement to ensure coverage in the event of an injury/incident; a student’s signature is required on this form. WEPA forms will be distributed by the Placement Coordinator, Requirements & Information, and must be signed and returned to the Placement Coordinator, Requirements & Information, Health Sciences Addition Room H138 prior to the start of each placement.

In the event of an injury/incident while attending a practice placement, the student must:

  1. Notify his/her clinical instructor or preceptor and receive first aid treatment
  2. Complete required reporting documentation for their placement site.
  3. With assistance of their clinical instructor, faculty advisor, and/or preceptor, complete the Western University Accident/Incident Report form and submit, within 24 hours of the incident, to:
