Request for Transfer Form

Policies and Procedures

Student must discuss and receive approval for transfer from supervisor and advisory committee.
Student must complete the correct transfer form.
Student must obtain supervisor’s signature on the form.
Student must submit the form to the HRS office 6 weeks prior to the start of term.
- Fall Term – Due in July
- Winter Term – Due in November
- Summer Term – Due in March
