To obtain your Access Code, go to studentservices.uwo.ca and go to “Login Help”. You will need to enter your date of birth, student and SIN numbers. If any information is missing from your file, and you are unable to get your Access Code, please call 519-661-2100. Complete online and email Amber Trent as well. If you are receiving income via UWO Payroll, the address must also be updated by following the instructions through Human Resources. Texts and materials can be purchased from Books Plus (plaza near the corner of Sarnia and Western Roads). The central Bookstore is located in Room 7, University Community Centre. You may be contacted in advance of the course start date with information such as the outline and/or textbook requirements. Typically you will be provided with this information the first day of class. See the Bookstore and Books Plus website for business hours. For full-time Society of Graduate Students (SOGS) members only—available in SOGS Office, Room 260 of the University Community Centre. Bring your student card. Bus passes will be available near the end of August. Check out the services and facilities. Students may request permission to take up to 2 half/one full course in other faculty at UWO, or another accredited university in Ontario (or Canada) as a “(Ontario) Visiting Graduate Student” or “(O)VGS.” Permission must be obtained from each institution or faculty involved. View the Student Services website. You will need your Student ID number and your 4-digit Access Code. Your Access Code is on your Offer of Admission or you can access the student services site and follow the steps provided to find your Access Code. Write it somewhere safe, as you will need it in the future, and this screen times out. Note that submission of grades for continuing students are not due to us from the instructors for two weeks into the following term. This means that grades will not be available for you to view online until approximately one month into the following term. View website for coverage and opt out information. The Health and Dental Plan Fee will be collected annually in September. This annual fee covers the Sept-Aug period. A wealth of information on banking, employment, travel and more can be found on website. The Allyn & Betty Taylor Library, located in the Natural Sciences Centre, is home to resources for the Faculty of Health Sciences. LOST AND FOUND The Campus Community Police Service is located in Room 57, Lawson Hall (formerly Stevenson-Lawson Building). Items lost in Elborn College are often handed in to the Health and Rehabilitation Sciences reception desk and held there for two weeks. Items are then forwarded on to the Campus Community Police. Online listings can be viewed at www.has.uwo.ca/housing/offcampus/index.htm. Prior to picking up your OSAP forms, please confirm that your OSAP loan has been released. View the Student Services website and log in by entering your student and Access Code to see if your loan has been received. All students picking up loan documents must present their Social Insurance Card and a valid government issued photo. This is a Ministry of Training, Colleges and Universities policy, and no exceptions will be made. It is also recommended that you bring your banking information to have your loans directly deposited into your bank. Loans will be distributed from Room 1120, Student Services Building. Western Parking and Visitor Services has moved to Room 4150, Support Services Building (use the Western Road entrance). They are open 8:30 am – 4:30 pm, Mondays to Fridays. You can pre-register for your pass online at www.uwo.ca/parking. Student card and vehicle ownership (photocopy accepted) are required for pickup. The parking lot at Elborn College is “Grey Perimeter” and you’ll need your pass before parking here. All Graduate Students are members of Society of Graduate Students which is governed by a council and executive board. They provide their members with benefits and services such as health and dental coverage, bus passes and financial assistance. Western One cards can be obtained from Room 1120, Student Services Building on the following dates/times: Sunday, September 5 - 12:00 pm to 3:00 pm; Monday, September 6 - 10:00 am to 3:00 pm. Beginning Monday, August 30th until Thursday, September 23rd, Student Central will have extended office hours: Monday, Tuesday, Thursday, and Friday - 9:00 am to 5:00 pm; Wednesday - 10:00 am to 5:00 pm. Workshops are available on various dates in August and September. To view the program and to register, please go to the website. Your tuition account is available for viewing through your student centre account. Graduate fees are due on September 8, 2011. Tuition payments can be made through a variety of ways. See the Registrar's Office for details. UHIP is mandatory health insurance coverage for non-Canadian residents and their families. Forms are to be picked up and completed in Room 1120, Student Services Building. If you have not done so, please activate your UWO email account. You will need your student number and Access Code.
