Abstract:

This cross-sectional mixed-method study comprehensively evaluated four primary health care models currently functioning in Ontario: Community Health Centres (CHC), Family Health Networks (FHN), Health Service Organizations (HSO), and Fee-for-Service (FFS) family practices. Data was collected from 137 randomly selected primary care sites using a patient survey, provider survey, practice survey, chart abstraction and interviews. Quantitative tools allowed for an evaluation of the performance (health care service delivery and clinical care) of the models while qualitative findings provided a deeper understanding of how the characteristics of the models can influence work processes and, ultimately, quality of care.

This project was funded by the Ontario Ministry of Health and Long-Term Care, Primary Health Care Transition Fund in the amount of $2,240,000.
