Request Form: Other Service Disruptions
Under the Accessibility Standards for Customer Service of Ontario, Western is to provide notice when there is a temporary disruption in services or facilities usually used by persons with disabilities to access the University.
Local Disruptions: Communicate Locally
Western departments are to communicate local disruptions (e.g. cancelled classes) directly to those affected through class emails, signage, departmental websites or other means and do not need to use this website.
All notices are to include:
1) the reason for the disruption
2) the expected duration
3) a description of alternative facilities or services, if available.
Use the Form below for Broad, Public Notification
Use this form if you need to provide notification to a wider audience (e.g. students, alumni, retirees, or other members of the public). Your request will be reviewed and posted on the Service Disruptions website. Communicate the disruption through local means as well.
