Below you will find several government and public sector resources providing information on senior and adult care to properly address your family's specific needs and the options and alternatives available to address those concerns.

  • If you are changing your address you may make that change online inMy Human Resources, or you may complete aPersonal Data Form.
  • If you are changing your name, please download and complete a Personal Data Formand mail or fax it to us at the address on the form. You may also scan and email it tohrhelp@uwo.ca. You will need to attach copies of two pieces of identification to support the change, including one piece of photo identification.
  • You may wish to add your new spouse to your benefits coverage and review your beneficiary designations. To do so, please complete the Benefit Application/Change form.
    Note: when adding/increasing life insurance, a medical questionnaire may be required if more than 31 days have passed since the first day of your eligibility or your spouse/dependent’s eligibility for the insurance. You may wish to contact Human Resource for assistance in understanding your current coverage, and the options available to you. To review your benefits information visit the Your Benefits website and choose your employee group.

  • Western's Human Resources has developed a Prenatal, Pregnancy and Adoption page that outlines the processes of applying for a leave after the arrival of your baby. The Life Events website through Service Canada offers detailed resources on how to register for your baby's birth certificate, social insurance number, OHIP, and passport.
  • You may add your child to your Extended Health and Dental plan using the Benefit Application/Change form.
  • You may wish to add or increase life insurance coverage and review your currentl life and pension beneficiary designations. Contact Human Resourcesfor assistaance reviewing your current coverage and coverage options available to you.

The EAP is available through the facilities of FSEAP Thames Valley / Family Service Thames Valley.
